Any student who participates in athletics and non-athletic activities in Chisholm High School will be charged an athletic and non-athletic activity fee as follows:

ATHLETICS                                                          NON-ATHLETICS

$90.00      Grades 9-12                                        $40.00

$70.00      Grades 8 and below                           $40.00

$350.00     Family limit                                        $350.00

Students who receive free lunch qualify for free athletic and non-athletic activity participation.

Students who receive reduced lunch qualify for a reduced athletic and non-athletic activity fee. 

Fees must be paid during registration or before the first contest or public appearance. 

Physical Examinations

Students registering for Athletics and Clubs must have a current physical examination on file in the athletic director's office before the student is authorized to participate in practice/tryouts.   To ensure clearance for participation, physicals should be completed and turned in to the athletic director no later than 3 school days prior to the first day of practice/tryouts.

Physicals are valid for 3 calendar years from physical exams date with a normal Annual Health Questionnaire. Download the physical form HERE.
